Your first visit to one of our offices involves a few special steps so that we can get to know you. Patients should login and complete the medical history registration through our patient portal.  If a patient is unable to complete the portal registration, we ask that the patient plan to arrive 15 minutes prior to their scheduled appointment to complete registration in our office.

Appointments

We know you have many choices when choosing an Otolaryngologist in Warwick, RI. Please contact our office (401) 921-5800 to schedule an appointment. If, for any reason, you cannot keep a scheduled appointment or will be delayed, please call as soon as possible. Charges may be incurred for appointments cancelled less than 24 hours before scheduled appointment time.


Payment Options

We accept a variety of healthcare insurances and all major credit cards.  ENT & Allergy requires 24 hours advanced notice to cancel an appointment.  If we do not receive a 24 hour notice for a scheduled appointment, we reserve the right to bill the patient a $50.00 no-show charge. 

At your initial appointment, please have your current insurance policy information and a photo ID available.  Some insurance companies require patients to have a referral from a primary care physician (PCP) in order for a patient visit to be covered. Please be aware of your insurance company's restrictions. Patients will need to bring an insurance card at the time of visit as well as a list of medications currently being taken. PCP's can fax referrals to our offices at: (401) 921-5826.

Referrals

Many insurance carriers require that patients obtain a referral/authorization from their primary care doctor when they seek treatment from a specialist. Please check with your insurance carrier to determine if your insurance carrier requires a referral/authorization. If one is required, please remember that you need to obtain one prior to your appointment.
